Pediatric Surgery:

Neonates and children are not merely small adults. Their bodies have different physiological needs and in the neonatal stage are carrying out various vital functions to stabilize in the new environment outside the womb. If at this significant time they require surgery for a defect or developmental anomaly, it would need to be in the hands of Surgeons, Anesthetists and Neonatal intensivists specially trained to handle such conditions at such a vulnerable time of their lives.

Specialized training and experience in Pediatric Surgery are vital.
The newborn infant with a major surgical condition should be transported and treated in a neonatal unit with Pediatric surgical facilities. It is only in such a unit that the specialized Surgical, Anesthetic, Nursing and Neonatal intensive care expertise and facilities necessary for neonatal surgery will be found.

Antenatal Diagnosis

  • High resolution USG during pregnancy has uncovered many fetal abnormalities which previously presented to the surgeon at birth
  • Indications for USG are mostly obstetric in nature and fetal anomalies are diagnosed serendipitously
  • 17 wks is the ideal time for early detection of fetal anomalies in mothers at risk

Fetal diagnosis

  • Antenatally diagnosed anomalies mandate delivery at a major maternity centre with full neonatal resuscitation services and advanced neonatal surgery backup.
  • USG is a safe and proven tool for diagnosis of anomalies while baby is in the womb. Whether detected or not in every case it is advisable to get delivery conducted at a centre under direct supervision of obstetrician and Neonatologist with high end NICU back up.
  • In case where anomaly is detected during antenatal period there is usually a need for timely work up & intervention e.g. - spina bifida where nerve function is preserved better after LSCS
  •  –earlier delivery in Gastroschisis to prevent gut from matting - & early neonatal surgery (within minutes) to reposit a  supple gut before edema sets in ( 20 minutes after birth )
  • Congenital Diaphragmatic Hernia cases may need a ventilator – so delivery only at a center with an advanced NICU & Neonatal Surgery.
